Kindly be notified that consistent with the Protection of Personal Information Act (POPIA), SA Quantum has adjusted the PEU Funeral Form for compliance.

Attached-herewith kindly receive the New Funeral Claim Form which has added information to be supplied, as compared to the other one.

The form has has been posted on the PEU Website for future access.

Implementation of the new form will be effected as from 01 June 2021.

We thus urge our structures to ensure that the form distributed widely to PEU Members.
